A Case Manager in the DDS Supported Living Program provides support to people who have a developmental disability and live in the community. Case Managers assist people with community activities, transportation, medications, and more.
The schedule for this position will be either Sunday-Thursday or Tuesday-Saturday between the hours of 8am and 8pm. This position also includes an emergency beeper rotation to provide people to support to people as needed during off hours.
This position requires travelling and transporting people in your personal vehicle within the Blackstone Valley/Central MA.
Assist in developing and implementing individualized service plans. Assist individuals to link to, manage and coordinate all community resources. Facilitate the development of valued community roles and relationships. Assist individuals with completing daily routines. Ensure health and safety of individuals and the cleanliness of their apartments. Implement all prescribed medical and medication protocols. Assist individuals to access and/or provide all needed transportation. Participate in team meetings, supervisions, and other assigned meetings. Review and complete all required documentation. Communicate pertinent information to stakeholders. Assist individuals to oversee their PCA Program, if applicable. Provide crisis intervention as needed and participate in regularly scheduled on-call beeper rotation. Acquire and maintain all required certifications Perform other related tasks as assigned.
B.A in Behavioral Sciences plus experience preferred. Alternative education and experience may be substituted for degree. High School Diploma, valid driver's license, and clean driver history required.
